To ensure a smooth and efficient consultation at our rheumatology practice, we kindly ask you to bring the following documents to your appointment:

Please bring your valid health insurance card, as this is required for billing purposes.
(This does not apply to privately insured or self-paying patients.)

If you are insured under the German statutory health system, please also bring a referral for specialized care (§116b SGB V).

In addition, any relevant previous medical records are very helpful for your evaluation. This includes:

• Medical reports
• Laboratory results
• Imaging studies (e.g. X-rays, MRI, CT scans)

Providing these documents allows us to better understand your medical history and to develop an appropriate and individualized treatment plan.
